Frequently Asked Questions (concerning the application process)

Question

I am a recently separated woman who has reverted to my maiden name. Whilst I have had my name amended for most of my documents, my Passport and Drivers Licence are still in my married name. Which name should I put in this section?

Answer

The name you put in section A will be the name that the Disclosure is issued in.

Assuming that you wish to have the Disclosure issued in the name you are currently using, it becomes an issue of documentation. If you have your original Birth Certificate (i.e. issued within 6 months of your birth) then we can accept this as a "List A" document and you can complete the form in your current name.

If you cannot produce a current Birth Certificate, you will need to provide 7 documents from "List B" (or 5 Documents from "List B" if you complete section E) in your current name.

Documents in your married name cannot be used for identity purposes for a Disclosure.

Question

I am working for a contractor in a school or hospital who are asking that I be checked. Who should I put in the Organisation Name?

Answer

Whilst the school or hospital is asking for you to be checked, they are actually asking your employer to have you checked and it is your employer's details that go on lines 14-19

Question

I have recently changed address and do not have any documentation for my new address?

Answer

If you moved very recently (less than 2 weeks) we can accept documents that show your previous address as written in Section D of the form.

If it is more than 2 weeks since you moved the CRB expects that you will have some evidence of the fact in the form of a rent-book, letter from your bank, credit-card company etc. We are required to see at least 1 document that shows your current address, although other addressed documents can show the previous address.

Question

All of the documentation relating to where I live is in my partner's or parents name?

Answer

Most people are registered with a doctor and the NHS Card shows both your date of birth and address.

If you are working, many payslips are addressed and you should have some documents from the Inland Revenue.

If you really don't have a listed document with your address on it you should call us on 0845 644 3298 to discuss what can be done.

Question

I'm going abroad soon and I don't want to send my passport or drivers licence.

Answer

We only retain documents for 48 hours maximum so allowing for the post each way your passport or drivers licence will only be out of your hands for up to a week.

If you are still not comfortable and have an original UK Birth Certificate (issued within 12 months of your birth) this is fine as a Group 1 document.

Finally, if you are only going to be abroad for a short time you can always ask the organisation requesting that you get a Disclosure if the application can wait until you return.

Question

I'm only 15 years old and don't have any of the documents listed.

Answer

Your parents should have a Child Benefit book which will have your child number on it. You should also have an NHS card in your own name. If you do not have an original UK Birth Certificate (issued within 12 months of your birth) or a passport please call us on 0845 644 3298 to discuss what can be done.
