Documentation Required

To support your application the Registered Body is required to see proof of identity and proof of address documentation to ensure that the applicant is the person being checked and the Disclosure will be delivered to the applicant. Without this check the application could be made by somebody other than the subject and the Disclosure sent to somebody else who has no right to the information.

All original documents sent to DDC will be returned in 2 working days by 1st class "Recorded Delivery" post.

APPLICANTS ARE REQUIRED TO PRODUCE A MINIMUM OF 3 DOCUMENTS AND APPLICATIONS CANNOT BE PROCESSED WITHOUT THEM BEING CHECKED

ALL DOCUMENTS SENT MUST BE ORIGINALS (photocopies are NOT acceptable).

The minimum acceptable combination is :-

1 document from Group 1 (below)

AND

2 documents from Group 2 (below)

If you cannot (or prefer not to) produce a document from Group 1, you can submit 5 documents from Group 2.

All documents must be in your current name (joint names is OK). At least one document must show your current address and at least one document must show your date of birth.

All documents must be dated within the time limits shown below.

ONLY ONE DOCUMENT OF ANY CATEGORY ALLOWED (i.e. Not 7 GCSE certificates)

Documents must be from different organisations e.g. a Nat West bank statement and a Nat West credit card statement would not be acceptable as 2 documents.

Group 1

• Valid Passport
• UK Driving Licence
• Original UK Birth Certificate (issued within 12 months of date of birth)
• Valid photo identity card (EU countries only)
• Valid Firearms Certificate
• Adoption certificate

Group 2

• Marriage Certificate
• Non-original UK birth certificate (issued after 12 months from date of birth)
• Non-UK birth certificate
• P45/P60 statement**
• Bank or building society statement*
• Utility bill (electricity, gas, water), landline telephone bill, mobile telephone contract/bill
• Valid TV licence
• Credit card statement*
• Store card statement*
• Mortgage statement**
• Valid insurance certificate,
• Correspondence/document from the Benefits Agency, the Employment Service, the Inland Revenue or a Local Authority*
• Financial Statement (e.g. Pension, endowment, ISA)**
• Valid vehicle registration document
• Mail order catalogue statement*
• Court summons**
• Valid NHS Card
• Addressed payslip
• National Insurance number card
• Exam certificate (e.g. GCSE, NVQ)
• Child benefit book**
• Connexions card
• Certificate of British nationality
• Work permit/visa

* this documentation should be less than 3 months old

**this document must have been issued within the past 12 months
