I have not received/lost my disclosure application form. What should I do?

Online applications.

If you’re waiting for your email with log-in details, our first suggestion is to check your junk/spam folders. They are often found in there as this will likely be the first email you’ve received from us. We recommend adding our domain @ddc.uk.net to your ‘safe senders’ or approved mailing list to ensure you’re getting our messages. If you’ve mislaid the original email but can remember your username then you can use the ‘lost password’ tools on the login page https://online.ddc.uk.net/ Your form will be secured with the security challenges and responses that you set when you first saved any information on to the form. If you can not remember any of these the unfortunately the only option is to reset your form and begin again. The organisation that requested the check can also re-issue the log-in email if you have never used them (i.e. the form is still blank).

Postal applications.

If you have not received your paper application form or envelope for sending your documents to us then there are a few options depending on the services your organisation are taking. If you’re completing an online form and expecting an envelope to return original document to us then please complete all your online steps. We will normally post your return envelope 2 – 3 working days after you have submitted your form details and chosen the identity documents you intend to provide. If you have already completed all the online steps and seen the confirmation screen, then it may be the case that the envelope has gotten lost in the post. My log-in details are not working:

If you have received your log-in email but the details do not appear to be working, then please copy and paste the username and password into the browser. The common issue we see is that applicant mis-type the details, rather than copying them directly into the log-in page. If you have re-requested details or tried to reset your log-in then please use the most recent email details sent over. As per the above, the reset email may take some time to be received by your junk/spam inbox.
